H1N1 Vaccine With Seasonal Flu: If the H1N1 Vaccine and the seasonal flu vaccine are given on the same day, the H1N1 vaccine must be billed with the 59 modifier.

Consultation Codes: Beginning January 1, 2010, Medicare will no longer recognize the use of CPT codes related to Consultation services in the ambulatory (office) or hospital settings.
  • Non-Dual Members (Medicaid Only Members) – Health Choice Arizona (HCA) will continue to accept claims submitted with dates of service 1/1/10 going forward with consultation codes 99241-99245, 99251-99255.

  • Dual Members – Medicare crossover claims denied by Health Choice Generations (HCG) or another Medicare provider for our dual eligible members will be denied by HCA for inappropriate coding. Providers must rebill to Medicare. Once HCA receives a properly coded Medicare crossover claim, we will then consider the claim for secondary reimbursement.

    Health Choice Arizona (HCA) is excited to offer this new resource for our providers!

    The HCA Provider Portal has been designed with you in mind. Our goal for the site is twofold.

    We want to assist you with reducing administrative burdens by streamlining processes and providing a one-stop-"resource shop" saving you time and money.

    We want to provide you with the tools you need so you can make the best decisions for your patients.

    Included in this site, and accessed through a secure portal, is patient data such as claims history and prior authorizations. In addition, you will find forms, newsletters, the HCA Provider Manual, continuing education information and links to other medical and dental information.

    Prior HealthChoiceAZ.com Account Holders

    In an ongoing effort to improve Health Choice's web-based services, we have not only designed and built a brand new Web site for Health Choice Arizona (HCA), but also are bringing to you a "single sign on" secure Provider Portal that allows access to both our Medicare (Health Choice Generations - HC Generations) and Medicaid (Health Choice - HCA) lines of business. The previous system was designed, built and hosted by an outside company. While satisfactory, it suffered from several drawbacks; drawbacks that prompted us to design and build this new "in-house" system which is vastly improved, more tightly integrated with our core business systems, and will expand with new features over time.

    One unfortunate fact of this transition is that all previous HCA account holders will be required to re-register - a process we have streamlined and made painless as possible - to use the new Provider Portal. For those Providers who are currently registered on our Generations Web site, you may use your HC Generations account to access the Provider Portal and follow the on screen prompts to register to view our Medicaid information. We apologize for this inconvenience and assure you this is a one time requirement.
